Anywhere you collect a phone number on a web form (usually in the "Contact Us" section) you will need to include this kind of SMS disclosure informing of consent to SMS messaging terms. Here is the example you can use to add to your web form on your website:
"By providing a telephone number and submitting the form you are consenting to be contacted by SMS text message from {your Business Name (DBA) here}. Message frequency may vary. Message & data rates may apply. Reply STOP to opt out of further messaging. Reply HELP for more information. See our Privacy Policy Terms & Conditions
You would need a terms and conditions page. This also needs to be a button located at the header or footer of the page and opens to a page with info.
Terms and Conditions must have an SMS disclosure that includes the types of messages consumers can expect to receive, texting cadence, message and data rate notices, privacy policy links, HELP information, and opt-out instructions.

- **Opt-Out** – Reply STOP at any time to unsubscribe. You will receive a confirmation message:
"You have now been opted-out and will receive no further messages from [Business Name] from this number. Reply START to resubscribe."
- **Help** – Reply HELP for assistance. You will receive:
"For additional information, please call [Business Name] at [Business support number]. To opt-out, reply STOP."
